This systematic review and narrative synthesis explores community-based models of care (MoCs) designed to support the clinical, functional, and personal recovery of individuals with persistent and complex mental health needs. The review evaluates a wide range of MoCs, including intensive case management, rehabilitation-focused programs, supported accommodation, and integrated community treatment. The findings highlight the importance of personal recovery goals, empowerment, and social connectedness in achieving positive outcomes. While established MoCs, such as intensive case management, demonstrate strong evidence for clinical and functional recovery, emerging MoCs focus more on personal recovery outcomes. The study underscores the need for continued innovation, co-design with individuals with lived experience, and further research to optimize the effectiveness of MoCs.
