Complete and upload the Supplemental Regulatory Questionnaire* (https://amtlb.alabama.gov/wp-content/uploads/2024/06/AMTLB-Supplemental-Regulatory-Questions-Fillable.pdf). Complete a minimum of 650 hours from an Accredited/Licensed Massage Therapy School. Out-of -State Schools are verified through NCBTMB or evidence of licensure by the state in which it is located. *Upload a copy of your transcript. Pass the MBLEx or NCBTMB Examinations. *Upload evidence of examination results. Evidence of $1,000,000 Professional and General Liability Insurance Coverage [Rule 532-X-3-.03(5)]. Application & License Fee for two-year license $100. Process payment online. Background check required. $25 fee paid separate from license fee and can only be done through this online application. Must have 16 CE Hours – certificates must be saved to upload. Do not attempt to upload certificates as separate files. *Upload one continuous file of certificates. Evidence of $1,000,000 Professional and General Liability Insurance Coverage [Rule 532-X-3-.03(5)]. Renewal Fee $100 for two-year license. Process payment online. Background check required. $25 fee paid separate from license fee and can only be done through this online application. No third-party background reports are accepted. Please check your email for the background check information.

Late fee $25 (expired less than one year), Reinstatement fee $75 (expired 1-5 years).

A massage therapist license expired more than five years may not be reinstated.
